To file a petition to make up missed work or to excuse a penalty, print and complete the form below. Bring the form and all supporting documents (such as a doctor's note for an illness or an obituary for a death in the family) to the Mathematics Learning Center within two days of the missed work. The decision of the Petition Review Panel is final; you will be notified of the decision by e-mail.
